The role of Quality Assurance is to ensure the delivery of high quality services to the individuals served by Athelas Institute. Quality Assurance is committed to ensure that the values, standards, and regulations of the State Developmental Disabilities Administration are incorporated into all services. The areas of focus are Health and Safety, Power and Choice, Community Integration, Informed Decision Making, and Supports for people with developmental disabilities.

| • Continuously conducting Site Reviews that monitor and evaluate the services provided by our Day and Residential Programs. | • Incident Report collection, review and investigation. |
| • Data collection and management that assesses trends and patterns to enchance quality services. | • Satisfaction Surveys completed Face to Face and by an outside consultant. |
| • Providing information and training to the individuals we support, their families and staff. | • Communicating with the individuals we support and their families, advocates, other providers, legislators and others. |
Quality Assurance continuously assesses for areas of improvment and develops a Plan of Action / Correction to ensure
that the needs and desires of the individuals that we serve are being met.
